The Following Documents are Required:
- Must be residents of one of the following zip codes: 20109, 20111, 20112. 20136, 20143, 20155, 20156, 20168, 20169, 20181, 20182, 22025, 22026, 22125, 22134, 22172, 22191, 22192, 22193, 22194, 22195)
- Provide proof of Residency
- Must be income-eligible at or below 300% of the 2022 Federal Poverty Guideline (See Chart Below)
- Must submit all supporting documents to prove a negative economic impact occurred as a result of COVID-19 pandemic or corresponding shutdowns (e.g., Loss of Income, Reduced Income, Increased Child Care Expenses, Increased Medical Expenses, Increased Rent/ Mortgage Payments)
- If you are not able to provide supporting documents you may use the self declaration forms below and upload to INFORMATION DOCUMENTS LINK
